Grand Horizons is located in a quiet and newly developed suburb of the city of Appleton. Residents of Grand Horizons enjoy a country-like setting in the city. Grand Horizons is a family-run CBRF with much to offer. Our home offers both spacious single and double rooms with a large, private bathroom with walk-in shower or wheelchair accessible. We invite our residents to decorate their rooms with their personal furnishings to create a home-like atmosphere as much as possible.

- Daily Meals - Three home cooked meals daily in a comfortable, spacious, open dining/kitchen. Snacks are available 24 hours a day.
- Experienced Full-time Registered Nurses during the day and on-call 24 hours a day.
- Quality, compassionate, personal care by experienced and attentive staff 24/7.
- Personal Care Services - Assistance with dressing, eating, bathing, grooming, toileting and mobility.
- Life Enrichment Programs.
- Housekeeping and Laundry Services.
- Health Monitoring by a licensed nurse: I&O, wt monitoring, vital signs, dabetic monitoring.

- Facility has a large screen LCD T.V. with basic cable.

- All utilities including free cable TV, except telephone service (available by private contract with residents and AT&T/SBC).
- Large single and double suites with private full baths.
- Ambulatory and non-ambulatory residents, all completely wheelchair accessible.
